NSQHS Standard 6 handover checklist

Use this as a practical checklist for repeatable handovers. Keep it short, make the owner explicit, and ensure the receiver can restate the plan.

  • Confirm patient identifiers and current location.
  • State the clinical goal for the next shift (what success looks like).
  • Call out high-risk problems first (deterioration, infection, bleeding, airway, delirium).
  • Explicit tasks with owner and time bound (by 20:00, before morning rounds).
  • Escalation criteria and who to contact.
  • Receiver confirms understanding or asks the one missing question.
